Burial Request Form

This form is to be completed for immediate needs by an individual requesting a burial plot for a deceased loved one. All information is used solely for cemetery records and coordination purposes. The completed form will be retained by the Lewis Cemetery Committee as part of its permanent records.

Please note: Full payment of all burial plot fees is required before any reservations are confirmed. Additionally, all burial open and closing fees must be settled in full before services can be provided on the burial day.
